Lewis T. Kulik, D.D.S, General Dentist and “In-Network” PPO Insurance Provider

Dr. Kulik received his Doctorate of Dental Surgery from New York University (NYU) and completed a dental internship at the Bronx-Lebanon Hospital Center.  He is a former Clinical Assistant Professor of Removable Prosthodontics at the NYU College of Dentistry.  His name has appeared in a number of Marquis Who's Who directories, including Who's Who in the East, Who's Who in Science and Engineering, Who's Who in the World, Who's Who in Medicine and Healthcare and Who's Who in America.  Dr. Kulik has also received the William K. Collins, Sr. Meritorious Service award and has, more than once, appeared in the "Guide to America's Top Dentists."  Dr. Kulik has completed fellowships with the Academy of General Dentistry, the International College of Dentists, the American College of Dentists, the Royal Society of Medicine and the Pierre Fauchard Academy.  He is a former member of the New York State Board of Dentistry and the Northeast Regional Board of Dental Examiners.  Dr. Kulik is currently a member of the American Dental Association, the Nevada Dental Association and the Southern Nevada Dental Society.

Blaine Hansen, D.D.S., Board Certified Orthodontist

Dr. Hansen graduated magna cum laude from Brigham Young University with a Bachelor of Science degree in Zoology.  He then pursued his dental degree at the University of Kentucky, where he graduated with high distinction, was admitted to Omicron Kappa Upsilon (the national dental honor society) and won several prestigious national academic awards.  Dr. Hansen continued his education at the University of Kentucky, where he earned his specialty certificate and master’s degree in orthodontics.

Dr. Hansen started his private practice in Las Vegas, where he delivers the highest quality orthodontic care.  He stays current in orthodontics by attending continuing education courses each year and participating in several professional associations.  He is a member of the American Association of Orthodontists, Nevada State Orthodontic Society, Pacific Coast Society of Orthodontists, American Dental Association and the Nevada Dental Association.  Dr. Hansen is a board-certified Diplomate of the American Board of Orthodontics: this prestigious achievement demonstrates outstanding performance in treatment and diagnosis of orthodontic patient care.

Dr. Hansen lives in Las Vegas with his wife, Katie, and their five children.  When he is not spending time with his family, he actively participates in his church and enjoys a variety of sports.

Graydon Briggs, D.D.S., General Dentist

Dr. Briggs limits his practice exclusively to root canals; he is not licensed as a specialist in Endodontics.  He graduated from the University of Southern California College of Dentistry in 1974.  He grew up in Salt Lake City, Utah, attending the University of Utah for his undergraduate studies.  Dr. Briggs practiced general dentistry in Salt Lake City, Utah, then began limiting his practice to root canals in Ogden, Utah.  He opened a general practice in Bicknell, Utah and maintained a practice limited to root canals in the offices of other general dentists.  In 2004 he sold his practice and now does root canals exclusively in Utah and Nevada for other dentists, in their respective offices.

Steven DeLisle, D.D.S., Dental Anesthesiologist

Dr. DeLisle received his undergraduate degree in Biochemistry from the University of San Diego in 2002.  He then pursued and attained his Dental degree from the University of Washington in 2007.  Following graduation Dr. DeLisle enrolled in Anesthesia Residency at Loma Linda University.  During his enrollment he completed rotations in Cardiology, Internal Medicine, and Medical Anesthesia at both Loma Linda University Medical Center and Riverside County Regional Medical Center.  He finished his training at the Special Needs Dental Clinic treating over 1,000 pediatric patients with special needs.  His patients include a diverse population ranging from those with mental or physical disabilities, to individuals with dental phobias.

Shivani Gupta, D.D.S., General Dentist, Family Dentist and “In-Network” PPO Insurance Provider

Dr. Shivani graduated from the University of Southern California, College of Dentistry in 2005.  Prior to this, she graduated with a BDS degree form MAHE University, Manipal, India in 1997.  Before moving to the US, she practiced dentistry for 5 years after starting up her own office in New Delhi.  She holds an active dental license both in the U.S. and India.

Dr. Shivani likes to cater to people of all age groups.  She accepts most PPO insurance plans.  Kindly call the office to get a complete listing of all the current accepted plans.  She currently lives in Las Vegas with her husband and two children.
